
Staying clear of diabetes, high blood pressure, and smoking through middle age may promote more than a decade of extra dementia-free years compared to peers carrying all three conditions. A large, long-running community-based study published in Neurology® Open Access and tracking 12,409 participants from four U.S. communities found that the gap in dementia-free years between the healthiest and least healthy midlife groups was 12.6 years, a number researchers say offers a clear, personal way to think about heart health and brain aging.
Dementia already looms large over American life. Roughly 42% of middle-aged adults in the U.S. may develop the condition during their lifetime, according to prior research cited in the study. Most conversations about preventing it focus on the odds of getting it at all, rather than something more personal: how many good cognitive years a person actually gets to enjoy. This study set out to answer that second question directly.
Researchers Followed 12,000 People for Nearly 30 Years
Researchers drew on data from the Atherosclerosis Risk in Communities study, a long-running project that began enrolling participants between 1987 and 1989 across four U.S. locations: Forsyth County, North Carolina; Jackson, Mississippi; the Minneapolis suburbs; and Washington County, Maryland. The team focused on 12,409 participants who were alive and free of dementia at age 55, with an average starting age of about 56. Just over half were women.
At a baseline checkup in the early 1990s, researchers noted whether each participant had diabetes, high blood pressure, or was a current smoker, then gave everyone a simple score from zero to three based on how many of those three conditions applied. From there, participants were tracked for a median of about 26 years, in some cases all the way to 2022. Over that stretch, 3,008 people developed dementia and 5,238 died without ever developing it.
Doctors did not rely on guesswork to identify dementia cases. Participants took cognitive tests in person, completed phone interviews when they could not make it to a visit, and had their hospital and death records checked continuously. An expert panel confirmed every diagnosis.
The More Risk Factors, the Fewer Good Years
A clear step-down pattern emerged. People with none of the three risk factors averaged 30.1 dementia-free years between ages 55 and 95. Add one risk factor and that number dropped to 26.3. Two dropped it to 21.0. All three brought it down to 17.5, a 12.6-year difference from the healthiest group. Only 176 participants landed in that highest-risk group, a small enough sample that the exact number carries more uncertainty than the figures for the larger groups.
Here is the twist: people with all three risk factors were nearly three times as likely to develop dementia as those with none, but they were more than five times as likely to die of something else first, before dementia ever got the chance to set in. By age 75, 10% of the highest-risk group had developed dementia, compared to just 3% of the healthiest group. By age 95, those numbers had essentially flipped, with the healthiest group reaching a 42% dementia rate while the highest-risk group topped out at just 23%. That is not because bad health protects against dementia. It is because the unhealthiest participants were, tragically, dying earlier from other causes.
Race and Sex Gaps in Dementia-Free Years Are Real
More risk factors meant fewer good years for every group researchers looked at, though the numbers were not identical across the board.
Women outlived men in dementia-free years at every risk level, averaging 18.1 years versus 16.6 for men among those with all three conditions. Women also had a higher lifetime rate of dementia by age 95, 40.3% compared to 32.8% for men, likely simply because they tend to live longer, giving the disease more years in which to appear.
Black participants had shorter dementia-free years than white participants at every risk level too. Among those with all three conditions, Black participants averaged 16.0 dementia-free years compared to 19.6 for white participants. Black participants also made up 60.2% of the highest-risk group despite representing a smaller share of the overall study population. Researchers say that heavier burden of health risk likely contributes to the disparity, and that addressing the social and economic forces behind it will matter just as much as clinical treatment.
People carrying a gene variant called APOE-ε4, which is linked to higher dementia risk, had fewer dementia-free years than non-carriers at every single risk level, from zero risk factors up through all three.
